Experienced Nurse Practitioners are needed to perform home health risk assessments on Medicare Advantage members throughout Hampden County, MA. This per diem opportunity allows clinicians to exercise autonomy by setting their own flexible schedules and traveling to various locations within the region, including Springfield, Chicopee, and Westfield. Each risk assessment visit lasts about 45 minutes and may include annual health evaluations or post-discharge visits, offering a rewarding chance to impact patient health directly within their homes.

Schedule:

• Flexible shifts available Monday through Sunday
• Ability to create your own schedule
• Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Conduct comprehensive home health risk assessments for Medicare Advantage plan members
• Review and assess patients prior and current medical history, active conditions, and medication usage
• Provide clear education to members regarding their health conditions and prescribed medications
• Identify any gaps in care and offer actionable recommendations to address them
• No requirement for developing treatment plans or prescribing medications

Qualifications:

• Current Nurse Practitioner license valid in Massachusetts
• Active CPR certification (BLS)
• Board certification through AANP or ANCC
• At least 6 months of recent nurse practitioner experience preferred
• MSN degree from an accredited nursing program

Successful candidates can expect compensation of $80$90 for each evaluation completed, in addition to a mileage stipend and paid administrative time.
