Experience the flexibility of per diem work while making a meaningful impact on patient care as a Nurse Practitioner specializing in home health risk assessments. In this role, youll conduct comprehensive evaluations for Medicare Advantage plan members throughout Miami-Dade County, including Miami, Hialeah, and Miami Gardens. Each visitapproximately 45 minutesmay consist of an annual health assessment or a post-discharge check-in, allowing you to offer vital insights into patients health in the comfort of their homes.

Key Responsibilities:
- Conduct annual in-home risk assessments for Medicare Advantage plan members
- Review and assess patients medical history, current health conditions, and medication use
- Educate patients on their medical conditions and medication regimens
- Identify care gaps and recommend follow-up actions to enhance patient well-being
- No prescribing or treatment planning required
Qualifications:
- Active Nurse Practitioner license in the state of Florida
- Current CPR certification (BLS)
- Board Certification from either AANP or ANCC
- Minimum of 6 months NP experience preferred
- MSN degree from an accredited nursing program
Compensation for this opportunity is highly competitive, with $80$90 provided per evaluation in addition to a mileage stipend and administrative time compensation.
