This per diem opportunity allows you to perform comprehensive risk assessments for Medicare Advantage plan members in a flexible, self-scheduled role covering areas such as Hollywood, Fort Lauderdale, and Pembroke Pines.

Schedule:

• Flexible shifts available Monday through Sunday
• Create your own working schedule
• Minimum weekly or bi-weekly hourly commitment as fits your availability

Key Responsibilities:

• Conduct thorough annual and post-discharge health risk assessments in patient homes
• Review each members medical history, current conditions, and medication use
• Educate patients on medical conditions and medications to promote better health outcomes
• Identify gaps in care and recommend appropriate next steps (without prescribing or creating treatment plans)
• Complete documentation for each visit, which typically lasts about 45 minutes

Qualifications:

• Active Nurse Practitioner License in Florida
• Current BLS/CPR certification
• Board Certification by AANP or ANCC
• Minimum six months of recent experience as a Nurse Practitioner (home health preferred)
• MSN degree from an accredited nursing program

As part of this role, you'll be compensated $80$90 per evaluation, with additional stipends for mileage and administrative time.
