In this per diem role, youll make a real impact by evaluating Medicare Advantage plan members in the comfort of their own homes across Hamilton County, including areas such as Chattanooga, East Ridge, and Soddy-Daisy.

Schedule:

• Flexible shifts available Monday through Sunday
• Design your own schedule to suit your availability
• Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Conduct in-home annual risk assessments for Medicare Advantage members
• Review and evaluate medical histories, current health status, and medication usage
• Educate patients about their conditions and medication regimens
• Identify gaps in care and recommend appropriate next steps for closure
• Visits last roughly 45 minutes, focusing on assessment rather than treatment or prescription
• No ongoing treatment planning or medication prescribing required

Qualifications:

• Active Nurse Practitioner license in Tennessee
• Current CPR certification (BLS)
• Board certified through AANP or ANCC
• Six months or more of recent NP experience preferred
• Master of Science in Nursing (MSN) from an accredited institution
