Nurse Practitioners with a passion for patient-centered care are needed to perform home health risk assessments on a per diem basis throughout Hamilton County, TN. Play a vital role in improving health outcomes for Medicare Advantage plan members residing in communities such as Chattanooga, East Ridge, and Soddy-Daisy.

Schedule:

• Flexible shifts available Monday through Sunday
• Set your own schedule
• Offers both weekly and bi-weekly minimum hourly commitments to fit your lifestyle

Key Responsibilities:

• Complete comprehensive annual risk assessments in patients' homes, spending about 45 minutes on each visit
• Review and evaluate patients current and prior medical histories, including medication and condition management
• Educate members on their medical conditions and medications to promote better self-care and health understanding
• Identify potential gaps in care and recommend actions to address them
• Assessments may include both annual health evaluations and post-discharge visits
• No requirement to generate treatment plans or prescribe medications

Qualifications:

• Active Nurse Practitioner license issued by Tennessee
• Current CPR certification (BLS)
• Board Certification through AANP or ANCC is essential
• MSN degree from an accredited nursing program required
• At least 6 months of recent experience as an NP is preferred
