Job Description

Deliver advanced patient care and make a meaningful impact in your local community through in-home health assessments. As a Nurse Practitioner specializing in Home Health, you will work per diem, conducting comprehensive risk assessments for Medicare Advantage plan members in Hillsville, VA, and throughout Carroll County. Locations may include Cana, Laurel Fork, and surrounding communities, supporting patients directly where they live.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Customize your own schedule
  • Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Perform annual and post-discharge home health risk assessments for Medicare Advantage members
  • Review prior and current medical history, evaluate existing conditions, and assess medication usage
  • Offer clear education regarding medical conditions and medication management
  • Identify gaps in care, making appropriate recommendations to support improved patient outcomes
  • No prescribing of medication or creation of treatment plans required

Qualifications:

  • Active Nurse Practitioner License in Virginia
  • Completion of MSN degree at an accredited nursing school
  • Current Board Certification (AANP or ANCC)
  • Minimum of 6 months current Nurse Practitioner experience preferred
  • Valid CPR certification (BLS)
