Take your Nurse Practitioner expertise into the community with a rewarding per diem opportunity focused on home health assessments. This autonomous position offers flexibility and a meaningful impact as you support Medicare Advantage plan members across Mercer County, NJ, including areas such as Trenton, Princeton, and Hamilton. The primary responsibility is conducting comprehensive home health risk assessments, with each visit averaging 45 minutes based on the needs of the patient, such as annual health checks or post-discharge follow-up.

Schedule:

• Enjoy flexible shifts available any day of the week, Monday through Sunday

• Meet a minimum commitment that fits your needs weekly or bi-weekly

Responsibilities:

• Complete thorough annual health risk assessments in patients homes
• Review current and past medical history, evaluate ongoing medical conditions, and verify medication usage
• Educate members about their medical conditions and prescribed treatments
• Identify possible gaps in care and deliver actionable recommendations for improvement
• No need to formulate treatment plans or prescribe medications, allowing you to focus on assessment and education

Qualifications:

• Active Nurse Practitioner license in New Jersey
• Current CPR certification (BLS)
• National board certification via AANP or ANCC
• MSN degree from an accredited nursing program
• Six months of recent NP experience is preferred

This position includes competitive compensation for each assessment completed, along with a mileage stipend and administrative time.
