Take on an impactful role in delivering healthcare directly to patients homes and enriching the standard of care for Medicare Advantage members. This per diem opportunity allows Nurse Practitioners with home health experience to perform crucial risk assessments and provide personalized patient education within Washington County, including areas such as Beaverton, Cornelius, Forest Grove, and Hillsboro.

Schedule:

• Flexible shifts available between Monday and Sunday
• Design your own schedule
• Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

• Conduct comprehensive home health risk assessments, averaging 45 minutes per visit
• Evaluate patients prior and current medical history, ongoing conditions, and medication usage
• Educate patients on their health conditions and medications, supporting quality self-care
• Identify gaps in healthcare and offer actionable recommendations for improvement
• Focus solely on assessmentno prescribing or treatment planning is involved

Qualifications:

• Active Nurse Practitioner license in Oregon
• Current CPR/BLS certification
• Board Certification through AANP or ANCC
• At least 6 months of recent Nurse Practitioner experience preferred
• Master of Science in Nursing (MSN) from an accredited program

This per diem position offers competitive compensation, with $80$90 provided per evaluation, in addition to a mileage stipend and coverage for administrative time. Youll enjoy significant autonomy by setting your own schedule and making a direct impact on patients health right in their homes.
