This role empowers you to provide vital home health risk assessments for Medicare Advantage plan members throughout Adams County, OH. As a key provider, your visits will help ensure comprehensive care for patients in Peebles, West Union, Manchester, and nearby communities, with assignments tailored to your availability each week.
Schedule:
- Flexible shifts offered Monday through Sunday
- Enjoy the ability to create your own schedule
- Minimum weekly or bi-weekly hourly commitment required
Responsibilities:
- Conduct thorough annual home health risk assessments for Medicare Advantage members
- Evaluate prior and current medical history, assess medical conditions, and review medication history
- Deliver patient education regarding medical conditions and medications
- Identify care gaps and recommend appropriate actions to improve patient outcomes
- Focus strictly on assessmentno requirement for developing treatment plans or prescribing medications
Qualifications:
- Valid Nurse Practitioner license in Ohio
- Current CPR certification (BLS)
- Board Certified by AANP or ANCC
- Minimum of six months experience as a practicing NP (preferred)
- Master of Science in Nursing (MSN) from an accredited program
