Job Description

Deliver high-quality, compassionate care directly to individuals in their homes with a meaningful per diem opportunity as a Nurse Practitioner. In this role, you will perform home health risk assessments for Medicare Advantage plan members residing within Highland County, including areas such as Hillsboro, Greenfield, and Leesburg. Each visit, averaging 45 minutes, may range from annual assessments to post-discharge follow-up visits. The position offers the autonomy to create your own schedule with flexible shifts throughout the week.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Self-scheduling design shifts to fit your life
  • Choose between minimum weekly or bi-weekly hourly commitments

Key Responsibilities:

  • Conduct annual home health risk assessments for Medicare Advantage members
  • Review prior and current medical history, medical conditions, and medication records
  • Educate patients on their diagnoses, treatments, and medications
  • Detect and address potential gaps in care with clear recommendations
  • No requirement for treatment planning or medication prescribing ensures a focused, assessment-based visit

Qualifications:

  • Active Nurse Practitioner license in Ohio
  • Current BLS (CPR) certification
  • National board certification (AANP or ANCC)
  • Minimum of 6 months recent NP experience (preferred)
  • Master of Science in Nursing (MSN) from an accredited institution
