Embrace a rewarding per diem opportunity delivering meaningful care and comprehensive assessments as a Nurse Practitioner specializing in home health. This role centers on empowering Medicare Advantage plan members throughout Adams County by conducting focused home health risk assessments in their homes, with visits lasting approximately 45 minutes. The scope of work ranges from annual health checkups to post-discharge visits, ensuring that patients in Peebles, West Union, Manchester, and nearby communities receive attentive, personalized care.
Schedule:
- Flexible shifts available seven days a week
- Choose your own work days and hours
- Commit to a minimum number of hours weekly or bi-weekly
Primary Responsibilities:
- Perform annual risk assessments for Medicare Advantage members in their residences
- Review each patients medical, surgical, and medication history
- Offer education on diagnoses and medications
- Detect gaps in care and suggest actionable next steps
- Focus solely on assessmentno prescribing, treatment planning, or follow-up is required
Qualifications:
- Active Nurse Practitioner license in Ohio
- Successful completion of a masters degree (MSN) from a recognized nursing program
- Board Certification via AANP or ANCC
- Valid CPR certification (BLS)
- At least six months of NP experience preferred
