Were seeking an experienced Nurse Practitioner for per diem opportunities focused on conducting home health risk assessments for Medicare Advantage plan members. Schedule:

• Flexible shifts offered between Monday and Sunday
• Create your own work schedule
• Opportunity for minimum weekly or bi-weekly hourly commitments

Responsibilities:

• Perform comprehensive annual home health risk assessments for Medicare Advantage members
• Assess prior and current medical history, existing conditions, and medication use
• Deliver patient education on chronic conditions and prescribed medications
• Identify care gaps and provide recommendations for follow-up, without the responsibilities of developing treatment plans or prescribing medications
• Complete each risk assessment in an average of 45 minutes
• Visits may include annual assessments or post-discharge check-ins, depending on patient needs

Qualifications:

• Current Nurse Practitioner license in Ohio required
• Active CPR/BLS certification
• Board certification via AANP or ANCC
• Minimum of six months recent Nurse Practitioner experience preferred
• Masters degree in Nursing (MSN) from an accredited program

Youll receive compensation of $80$90 per evaluation, which includes a mileage stipend and administrative time, rewarding both your time and expertise.
