Job Description
Take on an impactful role performing home health risk assessments for Medicare Advantage plan members across Monmouth County, including communities such as Middletown, Howell, and Marlboro. In this per diem opportunity, Nurse Practitioners visit patients in their homes to deliver crucial wellness evaluations and health education, supporting post-discharge recovery or annual review processes.
Schedule:
- Flexible shifts available Monday through Sunday
- Create your own schedule to accommodate work-life balance
- Minimum hourly commitment, selectable weekly or bi-weekly
Responsibilities include:
- Conducting comprehensive annual risk assessments in home settings
- Reviewing each patients medical history, current conditions, and medication regimen
- Providing clear education on diagnoses and prescriptions to enhance patient understanding
- Identifying gaps in care and communicating recommendations for follow-up or intervention
- Focusing exclusively on assessmentsno prescribing or treatment planning required
Preferred qualifications:
- Current and valid Nurse Practitioner license in New Jersey
- Active CPR certification (BLS required)
- Board Certification via AANP or ANCC
- Minimum of 6 months of recent NP experience (preferred)
- MSN degree from an accredited nursing program
