This per diem opportunity offers full control over your schedule and valuable work within Monmouth County, NJmeeting Medicare Advantage plan members where they reside.
Schedule/Hours:
- Flexible shifts available every day, Monday through Sunday
- Create your own schedule to match your needs
- Minimum hourly commitment on a weekly or bi-weekly basis
Key Responsibilities:
- Conduct annual risk assessments in patients homes, typically lasting 45 minutes per visit
- Evaluate patient medical history, conditions, and medication use
- Educate patients on their conditions and medication regimes
- Identify care gaps and recommend next steps for care optimization
- Activities focus on assessment and educationno medication prescribing or treatment planning required
Qualifications:
- Active Nurse Practitioner licensure in New Jersey
- Current CPR certification (BLS)
- Board Certified through AANP or ANCC
- Minimum of six months experience as a practicing NP preferred
- Master of Science in Nursing (MSN) from an accredited program
