Job Description

Deliver essential care directly to patients homes as a Nurse Practitioner specializing in home health risk assessments. In this flexible per diem role, you'll serve Medicare Advantage members throughout Alachua County, evaluating patients in Gainesville, Alachua, Newberry, and surrounding areas. Each visit averages 45 minutes and may involve an annual health assessment or a post-discharge review. Youll gain experience in a rewarding environment where provider autonomy and patient-centered care are valued.

Schedule:

  • Choose your own shifts between Monday and Sunday
  • Create your personalized weekly or bi-weekly schedule
  • Commitment requirements are minimal and easily tailored to your availability

Responsibilities:

  • Conduct comprehensive home health risk assessments for Medicare Advantage members
  • Evaluate current and prior medical history, health conditions, and medication records
  • Provide patient education on health conditions, preventive measures, and medication management
  • Identify care gaps and deliver actionable recommendations to address them
  • No requirement to create treatment plans or prescribe medications

Qualifications:

  • Active Nurse Practitioner license in Florida
  • Board Certified by AANP or ANCC
  • Master of Science in Nursing (MSN) from an accredited institution
  • Basic Life Support (BLS/CPR) certification
  • At least 6 months of current nurse practitioner experience (preferred)
