Job Description

Enjoy an impactful role providing advanced care in a home health setting as you conduct comprehensive risk assessments for Medicare Advantage members throughout Alachua County. This per diem opportunity is ideal for Nurse Practitioners who value flexibility and are committed to patient-centric healthcare delivery.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Self-scheduling for optimal work-life balance
  • Minimum commitment required on a weekly or bi-weekly basis

Key Responsibilities:

  • Perform in-home annual risk assessments for Medicare Advantage plan members
  • Evaluate patients medical histories, chronic conditions, and medication profiles
  • Offer clear education regarding diagnoses and medications to foster patient understanding
  • Identify care gaps and provide recommendations for clinical follow-up or resolution
  • Visits are typically 45 minutes and may include annual health reviews or post-discharge assessments
  • No prescribing or creation of treatment plans required

Qualifications:

  • Active Nurse Practitioner license in Florida
  • Board Certification via AANP or ANCC
  • Current CPR (BLS) certification
  • At least six months of recent experience as an NP preferred
  • MSN from an accredited nursing program

This per diem position offers competitive compensationearn $80-$90 per completed evaluation, enhanced by a mileage stipend and compensation for administrative time. You'll serve patients in Gainesville, Alachua, Newberry, and other communities within Alachua County, with patient assignments varying by shift.
