Job Description
Deliver high-quality, patient-centered care as a Nurse Practitioner specializing in home health risk assessments across Forsyth County, NC. In this per diem role, you will have the flexibility to tailor your schedule, including shifts between Monday and Sunday, and the ability to set your own hours with minimum weekly or bi-weekly requirements.
Youll play a crucial part in assessing Medicare Advantage plan members, focusing on locations including Kernersville, Clemmons, and Lewisville. Each home health risk assessment takes approximately 45 minutes and may include an annual health review or a post-discharge visit. Your expertise will support patients understanding of their medical conditions, ensure continuity of care, and help close gaps in their health needsall without responsibilities for creating treatment plans or prescribing medication.
Key Responsibilities:
- Conduct comprehensive, in-home risk assessments on Medicare Advantage members
- Review medical history and current conditions, including medications
- Deliver education to patients on their health conditions and prescribed therapies
- Identify gaps in care and provide actionable recommendations to patients and care teams
- Document findings and contribute to improved quality of care
Qualifications:
- Active Nurse Practitioner licensure in North Carolina
- Board Certification through AANP or ANCC
- Current BLS (CPR) certification
- MSN degree from an accredited nursing program
- Preferred: At least 6 months of nurse practitioner experience
This position compensates $80-$90 per evaluation, along with mileage stipends and pay for administrative time.
