Advance your career as a Nurse Practitioner specializing in home health with a per diem opportunity that offers autonomy and flexibility. In this role, you'll provide vital home health risk assessments to Medicare Advantage plan members across Guilford County, North Carolina, including areas such as Greensboro, Summerfield, Oak Ridge, and Stokesdale. Each assessment is scheduled for about 45 minutes, and assignments may include annual health evaluations or post-discharge visits. Scheduling is highly adaptable to fit your availability and preferences.
Schedule:
- Flexible shifts available between Monday and Sunday
- Self-scheduled shifts for maximum convenience
- Choose your minimum weekly or bi-weekly commitment
Key Responsibilities:
- Perform comprehensive annual home health risk assessments for Medicare Advantage members in their homes
- Review and document patients medical history, current conditions, and medications
- Deliver education to patients on managing their health, including medical conditions and medication usage
- Detect gaps in care and offer actionable recommendations to improve patient outcomes
- No treatment planning or prescription duties required
Qualifications:
- Active Nurse Practitioner license in North Carolina
- Current BLS/CPR certification
- Board certified by AANP or ANCC
- Six months of recent nurse practitioner experience preferred
- MSN degree from an accredited nursing institution
