Job Description
Seeking a dedicated Nurse Practitioner to provide home health risk assessments on a per diem basis. This role involves visiting Medicare Advantage plan members in Alexander County, including Bethlehem, Taylorsville, and Hiddenite, to conduct comprehensive evaluations. Each assessment takes approximately 45 minutes and may be related to annual health reviews or post-discharge follow-ups. With a highly flexible schedule, you can create your own shifts between Monday and Sunday, meeting minimum hourly commitments weekly or bi-weekly.

Key Responsibilities:
- Perform annual and post-discharge home health risk assessments for Medicare Advantage members
- Review and document previous and current medical history, existing conditions, and medication usage
- Offer patient education on diagnoses and medication regimens
- Detect care gaps and suggest actionable recommendations
- Note: No prescribing or treatment plan development required
Qualifications:
- Active Nurse Practitioner license in North Carolina
- CPR certification (BLS) required
- Board-certified through AANP or ANCC
- Minimum of 6 months' recent NP experience preferred
- MSN degree from an accredited nursing program
Candidates will travel to members' homes in various areas within Alexander County during each shift. A commitment to compassionate, thorough patient care and strong documentation is essential.
