Job Description
Deliver essential healthcare directly to patients homes as a dedicated Nurse Practitioner supporting Medicare Advantage plan members. In this per diem position, youll perform comprehensive home health risk assessments for individuals throughout Lincoln County, including Iron Station, Lowesville, Westport, Lincolnton, and surrounding communities.
Schedule:
- Flexible shifts available any day of the week (Monday Sunday)
- Choose your own schedule to fit your lifestyle
- Minimum weekly or bi-weekly hourly commitments
Responsibilities:
- Conduct thorough annual home health risk assessments for Medicare Advantage participants
- Review patients prior and current medical history, conditions, and medication usage
- Provide clear health education about diagnoses, medications, and at-home care
- Identify gaps in care and recommend steps to improve health outcomes
- Focus exclusively on assessmentno prescribing or treatment planning required
Qualifications:
- Active Nurse Practitioner License in North Carolina
- Board Certification via AANP or ANCC
- Current CPR (BLS) certification
- MSN degree from an accredited nursing program
- At least 6 months of relevant NP experience preferred
