Job Description

A healthcare provider is seeking experienced Nurse Practitioners to perform home health risk assessments on a per diem basis. This role involves visiting Medicare Advantage members at their residences for comprehensive evaluations, focusing on annual health reviews or post-discharge visits. Candidates will serve patients throughout Miami-Dade County, including areas such as Miami, Hialeah, and Miami Gardens.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Create your own schedule based on availability
  • Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

  • Conduct thorough home health risk assessments for Medicare Advantage plan members
  • Review and document prior/current medical history, existing medical conditions, and medication usage
  • Educate patients about their health conditions and prescribed medications
  • Identify gaps in care, and make recommendations to help address them
  • No treatment plan development or prescription authority is needed

Qualifications:

  • Active Nurse Practitioner license in Florida
  • Current CPR/BLS certification
  • Board certified through AANP or ANCC
  • Minimum of 6 months current nurse practitioner experience preferred
  • MSN degree from an accredited school of nursing
