Job Description

An exciting per diem opportunity is available for an experienced Nurse Practitioner with a passion for home health. This role involves conducting comprehensive home health risk assessments for Medicare Advantage plan members within Minidoka County, including areas such as Rupert, Heyburn, and Paul. Each evaluation typically lasts around 45 minutes and encompasses annual health assessments as well as post-discharge check-ins. Schedule:

  • Flexible shifts available any day, Monday through Sunday
  • Choose shifts that fit your availability
  • Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

  • Complete home health risk assessments in patients residences throughout Minidoka County
  • Review prior and current medical history, assess existing conditions, and evaluate medication usage
  • Educate patients about their health conditions and medication regimens
  • Identify potential gaps in care and recommend action steps
  • No responsibility for treatment plan development or prescribing

Qualifications:

  • Active Nurse Practitioner license in Idaho (ID)
  • Current Basic Life Support (BLS) CPR certification
  • Board Certification through AANP or ANCC
  • At least six months of recent experience as a Nurse Practitioner preferred
  • Master of Science in Nursing (MSN) from an accredited program
