Seeking an experienced Nurse Practitioner to deliver high-quality home health risk assessments for Medicare Advantage plan members throughout Cassia County. This per diem opportunity is designed for independent, motivated professionals who excel in patient interaction and appreciate flexible work arrangements. The role focuses on comprehensive health evaluations conducted in patient residences across locations including Burley, Oakley, and Declo, with each visit averaging 45 minutes. The responsibilities may encompass annual assessments or post-discharge evaluations, depending on patient needs within the county.
Schedule:
- Flexible shifts available seven days a week
- Ability to design your own work schedule
- Minimum weekly or bi-weekly hourly commitment expected
Responsibilities:
- Perform thorough annual home health risk assessments in patient homes for Medicare Advantage plan members
- Review and document previous and current medical history, conditions, and medications
- Educate patients on their medical conditions, medication usage, and wellness practices
- Recognize gaps in care and recommend actionable steps to address them
- No requirement for treatment planning or prescription of medications
Preferred Qualifications:
- Valid Nurse Practitioner license in Idaho
- Active CPR (BLS) certification
- Board Certification through AANP or ANCC
- At least six months of current NP experience preferred
- MSN from an accredited nursing school
