Job Description

Deliver essential care and assessments to patients in their homes as a Nurse Practitioner specializing in home health. This per diem opportunity involves conducting comprehensive risk assessments within a variety of settings across Chowan County, providing important evaluations for Medicare Advantage plan members. Visits typically last about 45 minutes and may include annual health assessments or post-discharge follow-ups.

Key Responsibilities:

  • Perform annual home health risk assessments for Medicare Advantage members
  • Assess medical histories, current health conditions, and medication use
  • Offer education to patients regarding health conditions and medication management
  • Identify care gaps and suggest solutions to promote better health outcomes
  • Visits focus on assessmentno treatment planning or medication prescribing required

Qualifications:

  • Active Nurse Practitioner license in North Carolina
  • Current CPR (BLS) certification
  • Board certification via AANP or ANCC
  • Minimum of 6 months NP experience is preferred
  • Master of Science in Nursing from an accredited institution

You will experience a significant degree of autonomy in this role, empowering you to tailor your schedule and work in a variety of communities, including Edenton, Tyner, Ryland, and Valhalla. In recognition of your expertise and time, compensation will be provided at $80-$90 per evaluation, along with a mileage stipend and administrative time.
