This per diem opportunity offers you the flexibility to set your own schedule, with shifts available any day of the week and a minimum hourly commitment weekly or bi-weekly.

Key Responsibilities:

• Conduct comprehensive home health risk assessments for Medicare Advantage plan members
• Review and evaluate patients prior and current medical histories, ongoing conditions, and medication records
• Educate patients about their medical conditions and appropriate medication usage
• Identify any existing gaps in care, offering recommendations to ensure optimal patient health outcomes
• Visits may include annual health assessments or post-discharge follow-ups
• Focus exclusively on evaluationno treatment plans or prescription authority required

Required Qualifications:

• Current and unrestricted Nurse Practitioner License in Texas
• Active CPR certification (BLS)
• Board certification through AANP or ANCC
• Master of Science in Nursing (MSN) from an accredited nursing program
• At least 6 months of recent Nurse Practitioner experience preferred

This role features compensation of $80$90 per evaluation, along with a mileage stipend and paid administrative time.
