Deliver essential care directly to patients in their homes as a Nurse Practitioner specializing in home health assessments in Henderson County. This per diem opportunity allows you to make a difference by evaluating and educating Medicare Advantage plan members while enjoying complete scheduling flexibility.
Schedule:
- Flexible shifts available Monday through Sunday
- Customize your own schedule
- Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Perform annual and post-discharge home health risk assessments for Medicare Advantage members
- Review each patients medical and medication history, as well as ongoing health conditions
- Educate patients regarding their diagnoses and prescribed medications
- Identify and address gaps in care, providing recommendations to promote optimal health outcomes
- Documentation and professional communicationtreatment plan development and prescribing are not required
Qualifications:
- Active Nurse Practitioner license in North Carolina
- Current CPR certification (BLS)
- Board certified via AANP or ANCC
- Master of Science in Nursing (MSN) from an accredited program
- At least 6 months of recent experience as an NP strongly preferred
