Job Description

Deliver essential care directly to patients in their homes as a Nurse Practitioner specializing in home health assessments in Henderson County. This per diem opportunity allows you to make a difference by evaluating and educating Medicare Advantage plan members while enjoying complete scheduling flexibility.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Customize your own schedule
  • Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

  • Perform annual and post-discharge home health risk assessments for Medicare Advantage members
  • Review each patients medical and medication history, as well as ongoing health conditions
  • Educate patients regarding their diagnoses and prescribed medications
  • Identify and address gaps in care, providing recommendations to promote optimal health outcomes
  • Documentation and professional communicationtreatment plan development and prescribing are not required

Qualifications:

  • Active Nurse Practitioner license in North Carolina
  • Current CPR certification (BLS)
  • Board certified via AANP or ANCC
  • Master of Science in Nursing (MSN) from an accredited program
  • At least 6 months of recent experience as an NP strongly preferred
