Deliver comprehensive, patient-centered care by performing home health risk assessments for Medicare Advantage plan members. In this per diem role, you will visit members in their residences throughout Buncombe County, conducting assessments that support health and wellness in settings ranging from Asheville to Montreat, Biltmore, and Woodfin. Each home visit averages about 45 minutes and may involve either an annual health assessment or a post-discharge evaluation. This position is uniquely flexible, allowing you to define your availability and commitment.
Schedule:
- Flexible shifts available Monday through Sunday
- Freedom to create your own schedule
- Choose your minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Conduct annual home health risk assessments for Medicare Advantage members
- Review current and past medical history, conditions, and medication regimens
- Provide education on disease management and prescribed medications
- Identify gaps in care and offer recommendations to close those gaps
- No requirement to prescribe medication or develop treatment plans
Qualifications:
- Active Nurse Practitioner license in North Carolina
- Current CPR certification (BLS)
- Board certification through AANP or ANCC
- At least 6 months of recent Nurse Practitioner experience preferred
- MSN degree from an accredited nursing program
