Take on a vital and impactful role by providing comprehensive home health risk assessments for Medicare Advantage members in Madison County, TN. As a skilled Nurse Practitioner, you will visit patients in various locations including Jackson, Medon, and Three Way. This per diem opportunity offers exceptional flexibility with a self-managed schedule and the ability to shape your own work commitments.
Schedule:
- Flexible shifts available any day, Monday through Sunday
- Create your own schedule
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Perform thorough annual home health risk assessments in patients' homes
- Review and evaluate prior and current medical histories, health conditions, and medication usage
- Educate patients on medical conditions and medication regimes
- Identify gaps in care and recommend appropriate next steps for follow-up
- No requirement to develop treatment plans or prescribe medicationsthe focus is on assessment and education
Desired Qualifications:
- Active Nurse Practitioner license in Tennessee
- Board Certification (AANP or ANCC)
- Current BLS/CPR certification
- MSN from an accredited nursing program
- Minimum of 6 months' experience as a practicing NP preferred
Compensation & Perks:
- Earn $80-$90 per completed evaluation, plus a mileage stipend
