Job Description

Advance your clinical expertise as a Nurse Practitioner providing home health risk assessments for Medicare Advantage plan members across Warren County, NC. Schedule/Hours:

  • Flexible shifts available any day, Monday through Sunday
  • Set your own work schedule
  • Choose minimum weekly or bi-weekly hourly commitments that fit your lifestyle

Primary Duties:

  • Perform comprehensive annual or post-discharge home health risk assessments, averaging 45 minutes per visit
  • Review and evaluate patients health history, current conditions, and medications
  • Educate patients on their medical conditions and prescribed medications
  • Identify and discuss any care gaps, providing clear recommendations for follow-up
  • No responsibility for prescribing medications or developing treatment plans

Preferred Qualifications:

  • Active Nurse Practitioner license in North Carolina
  • Board Certification via AANP or ANCC
  • Master of Science in Nursing (MSN) from an accredited institution
  • Current BLS/CPR certification
  • Minimum six months of recent Nurse Practitioner experience in a clinical setting
