Advance your clinical expertise as a Nurse Practitioner providing home health risk assessments for Medicare Advantage plan members across Warren County, NC. This per diem opportunity offers outstanding flexibility and the chance to make a meaningful impact by supporting patients in Norlina, Warrenton, Macon, and surrounding communities.
Schedule/Hours:
- Flexible shifts available any day, Monday through Sunday
- Set your own work schedule
- Choose minimum weekly or bi-weekly hourly commitments that fit your lifestyle
Primary Duties:
- Perform comprehensive annual or post-discharge home health risk assessments, averaging 45 minutes per visit
- Review and evaluate patients health history, current conditions, and medications
- Educate patients on their medical conditions and prescribed medications
- Identify and discuss any care gaps, providing clear recommendations for follow-up
- No responsibility for prescribing medications or developing treatment plans
Preferred Qualifications:
- Active Nurse Practitioner license in North Carolina
- Board Certification via AANP or ANCC
- Master of Science in Nursing (MSN) from an accredited institution
- Current BLS/CPR certification
- Minimum six months of recent Nurse Practitioner experience in a clinical setting
