Job Description

Deliver high-quality home health care in Vance County as a Nurse Practitioner dedicated to performing risk assessments for Medicare Advantage plan members. In this per diem role, you'll travel to patient homes in Henderson, Kittrell, Middleburg, and surrounding communities. Each visit averages 45 minutes, where youll complete either an annual evaluation or a post-discharge assessment, helping ensure continuity of care and improved health outcomes for each member you see.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Freedom to create your own schedule
  • Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

  • Conduct comprehensive annual home health risk assessments
  • Review and assess prior and current medical history, conditions, and medications
  • Educate patients on their diagnoses and treatment regimens
  • Identify care gaps and offer practical recommendations
  • Focus exclusively on assessmentno need for prescription management or development of new treatment plans

Qualifications:

  • Active Nurse Practitioner license in North Carolina
  • Board Certification through AANP or ANCC
  • Valid BLS/CPR certification
  • MSN degree from an accredited nursing program
  • At least six months of recent Nurse Practitioner experience is preferred
