Job Description
Deliver essential healthcare services in the community setting through flexible home health risk assessments. As a Nurse Practitioner specializing in home health, you will meet Medicare Advantage plan members in their residences, performing thorough assessments to ensure their continued well-being and addressing any care gaps.
Schedule:
- Flexible shifts available seven days a week
- Self-schedulingchoose your own hours
- Select from weekly or bi-weekly shift commitments
Key Responsibilities:
- Perform comprehensive home health risk assessmentsmost completed in 45 minutes
- Review patient medical histories, chronic conditions, and prescribed medications
- Provide education regarding medication regimens and disease management
- Identify and report gaps in care, recommending solutions to enhance patient outcomes
- Visits may include annual health assessments and post-discharge follow ups
- No requirement to develop treatment plans or prescribe medication
Qualifications:
- Active Nurse Practitioner license in Arkansas
- Current BLS/CPR certification
- National board certification (AANP or ANCC)
- Master of Science in Nursing (MSN) from an accredited program
- Minimum of 6 months experience as a practicing Nurse Practitioner preferred
