Nurse Practitioners with experience in home health are sought for a dynamic, per diem opportunity conducting home health risk assessments throughout Lee County. In this impactful role, you will provide in-depth evaluations for Medicare Advantage members, helping to identify care gaps, review medical histories, and educate patients on managing their healthall within the comfort of their homes. Assessments may take approximately 45 minutes, and you may be scheduled in locations such as Marianna, Moro, and Rondo within Lee County.
Schedule:
- Flexible shifts available seven days a week
- Self-schedulingchoose shifts that fit your availability
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Conduct annual health risk assessments and some post-discharge visits
- Review and evaluate patient medical histories, current conditions, and medications
- Offer education to patients regarding their conditions and medications
- Identify and recommend action for any gaps in patient care
- Strictly assessment-focused; no treatment planning or prescribing required
Qualifications:
- Active Nurse Practitioner license in Arkansas
- Current CPR certification (BLS)
- Board Certification through AANP or ANCC required
- MSN degree from an accredited nursing school
- At least six months of current NP experience preferred
