A rewarding opportunity is available for a Nurse Practitioner with experience in home health to support Medicare Advantage plan members by conducting risk assessments in Wapello County and surrounding communities. This flexible, part-time contract role provides autonomy in your schedule and the chance to make a significant impact on patient care in the comfort of their homes.
Scheduling Details:
- Flexible work hourscreate your own schedule
- Shifts available between Monday and Sunday
- Options to commit weekly or bi-weekly for hours based on your availability
Key Responsibilities:
- Conduct comprehensive annual home health risk assessments for Medicare Advantage members
- Review and evaluate current and prior medical histories, ongoing conditions, and medication lists
- Provide education to members regarding their medical conditions and medications
- Identify any care gaps and recommend solutions to enhance health outcomes
- No requirement to create treatment plans or prescribe medications
Qualifications & Experience Sought:
- Active Nurse Practitioner license in Iowa
- Board Certification via AANP or ANCC
- Current CPR certification (BLS)
- MSN degree from an accredited nursing program
- At least one year of NP experience (highly preferred)
- Ability to work independently and provide compassionate patient education
