This per diem opportunity is designed specifically for Nurse Practitioners who excel in independent settings and are passionate about preventive care in Berkeley County and its surrounding areas. You'll meet Medicare Advantage plan members where they arewhether in Martinsburg, Inwood, Hedgesville, or elsewhere locallyoffering quality assessments in the comfort of their own homes.

Schedule:

• Flexible shifts available Monday through Sunday
• Self-scheduling: create a work schedule that fits your availability
• Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

• Perform annual and post-discharge home health risk assessments for Medicare Advantage members
• Thoroughly review prior/current medical history, existing conditions, and medication usage
• Offer education to patients about their health status and prescribed medications
• Identify gaps in care and recommend actionable solutions
• No prescribing or creation of treatment plans required

Qualifications:

• Active and unrestricted Nurse Practitioner license in West Virginia
• Current CPR certification (BLS)
• Board certification through AANP or ANCC
• At least 6 months of clinical NP experience (preferred)
• MSN degree from an accredited nursing program

This role offers a competitive rate of $80-$90 per completed evaluation, as well as a mileage stipend and compensation for administrative time.
