Step into a dynamic per diem opportunity supporting Medicare Advantage members with comprehensive home health risk assessments. This role places you at the forefront of proactive healthcare, visiting patients in Hearne and surrounding areas within Robertson County, including Franklin, Calvert, and Bremond. As a Nurse Practitioner specializing in home health, your expertise will help bridge gaps in care and educate individuals for better health outcomesall without the need to prescribe medications or create detailed treatment plans.
Schedule:
- Flexible shifts available Monday through Sunday
- Set your own availabilitytailor your schedule to suit your needs
- Commitment can be weekly or bi-weekly according to your preference
Key Responsibilities:
- Complete thorough annual and post-discharge home health risk assessments for Medicare Advantage participants
- Obtain and evaluate patients previous and current medical history, ongoing conditions, and medication lists
- Educate members regarding their medical issues and medication regimens in the comfort of their homes
- Identify potential care gaps and recommend appropriate next steps for patients and primary care teams
- Focus visits on risk assessmentno prescriptive authority or treatment plan formulation required
Qualifications:
- Active Nurse Practitioner license in Texas
- Current CPR (BLS) certification
- MSN degree from an accredited nursing program
- Board certification via AANP or ANCC
- Minimum of six months of recent NP experience preferred
As part of this per diem position, you will earn $80$90 per evaluation, with an additional mileage allowance and compensation for administrative time.
