Experience a rewarding opportunity as a Nurse Practitioner specializing in home health risk assessments, serving Medicare Advantage plan members throughout Williamsburg, Jamestown, and the greater James City County area. Schedule:
- Flexible shifts available Monday through Sunday
- Design your own schedulechoose when you work
- Commitment required: minimum hours weekly or bi-weekly
Key Responsibilities:
- Conduct annual and post-discharge home health risk assessments (approximately 45-minute visits)
- Assess and document prior and current medical history, conditions, and medications
- Educate members about their conditions and proper medication usage
- Identify any care gaps and offer practical recommendations
- Note: No treatment plan creation or medication prescribing required
Qualifications:
- Active Nurse Practitioner License in Virginia
- Current CPR certification (BLS required)
- Board Certification via AANP or ANCC
- Minimum of 6 months current Nurse Practitioner experience preferred
- MSN degree from an accredited nursing program
