Job Description

Make a meaningful impact in patient care by performing comprehensive home health risk assessments for Medicare Advantage plan members in Kootenai County. This per diem opportunity allows Nurse Practitioners to leverage their clinical expertise while enjoying flexible scheduling and autonomy.

Schedule:

  • Flexible shifts available any day between Monday and Sunday
  • Self-schedulingchoose shifts that fit your availability
  • Minimum shift commitment on a weekly or bi-weekly basis

Key Responsibilities:

  • Conduct annual health assessments in members homes within designated areas
  • Assess prior and current medical histories, review medical conditions, and evaluate medication usage
  • Deliver clear education on health conditions and prescribed medications
  • Identify gaps in care, offering recommendations to address them
  • This role does not require prescribing medications or creating treatment plans

Qualifications:

  • Active Nurse Practitioner license in Idaho
  • Current BLS (CPR) certification
  • Board Certification via AANP or ANCC required
  • At least six months of recent clinical experience as a Nurse Practitioner preferred
  • MSN degree from an accredited nursing program

Nurse Practitioners are scheduled for visits across Kootenai County, including Coeur dAlene, Post Falls, and Hayden, ensuring a varied and engaging caseload. Each home health risk assessment typically lasts about 45 minutes and may include annual health evaluations or post-discharge assessments.

Compensation for this position is highly competitive, with rates of $80$90 per completed assessment. In addition, clinicians will receive a mileage stipend and compensation for administrative time.
