In this position, youll leverage your medical expertise to conduct risk assessments for Medicare Advantage plan members, supporting their healthcare journeys right in their homes. Schedule:

• Flexible shifts available Monday through Sunday

• Minimum weekly or bi-weekly hourly commitment ensures consistency while offering autonomy

Key Responsibilities:

• Perform annual home health risk assessments and post-discharge visits, each approximately 45 minutes in duration.
• Travel within Alachua County to serve areas including Gainesville, Alachua, and Newberry.
• Review and evaluate prior and current medical history, ongoing conditions, and medications.
• Provide patient education on medical conditions and prescribed medications, empowering patients in their care.
• Identify gaps in care, making recommendations to enhance overall patient well-being.
• This role does not require treatment planning or medication prescribing.

Qualifications:

• Valid Nurse Practitioner License in Florida required.
• MSN degree from an accredited nursing school.
• Board Certification through AANP or ANCC.
• Current CPR certification (BLS).
• At least 6 months of recent nurse practitioner experience preferred.
