Embark on a rewarding per diem opportunity, serving communities throughout Alachua County with a home health role focused on comprehensive patient assessments. In this position, youll leverage your medical expertise to conduct risk assessments for Medicare Advantage plan members, supporting their healthcare journeys right in their homes. Schedule:
- Flexible shifts available Monday through Sunday

- Minimum weekly or bi-weekly hourly commitment ensures consistency while offering autonomy
Key Responsibilities:
- Perform annual home health risk assessments and post-discharge visits, each approximately 45 minutes in duration.
- Travel within Alachua County to serve areas including Gainesville, Alachua, and Newberry.
- Review and evaluate prior and current medical history, ongoing conditions, and medications.
- Provide patient education on medical conditions and prescribed medications, empowering patients in their care.
- Identify gaps in care, making recommendations to enhance overall patient well-being.
- This role does not require treatment planning or medication prescribing.
Qualifications:
- Valid Nurse Practitioner License in Florida required.
- MSN degree from an accredited nursing school.
- Board Certification through AANP or ANCC.
- Current CPR certification (BLS).
- At least 6 months of recent nurse practitioner experience preferred.
