Job Description

Deliver patient-centered care as a Nurse Practitioner specializing in home health risk assessments for Medicare Advantage members across Craven County, including Havelock, Vanceboro, and Bridgeton. This per diem opportunity offers the freedom to create your own flexible schedule, with shifts available throughout the week, Monday to Sunday. Enjoy the diversity of conducting visits that range from annual health assessments to post-discharge follow-ups, lasting approximately 45 minutes each.

Schedule:

  • Minimum commitment in weekly or bi-weekly hours

Key Responsibilities:

  • Conduct comprehensive home health risk assessments for Medicare Advantage plan members
  • Review and document patients prior and current medical history, medical conditions, and medication usage
  • Educate patients on their health conditions and prescribed medications
  • Identify gaps in care and present recommendations to improve patient outcomes
  • Visits are strictly assessment-focusedno treatment planning or medication prescriptions required

Qualifications:

  • Active Nurse Practitioner license in North Carolina
  • Current BLS/CPR certification
  • Board Certification through AANP or ANCC
  • MSN from an accredited nursing school
  • Minimum of six months current experience as a Nurse Practitioner (preferred)
