A per diem opportunity is available for a skilled Nurse Practitioner to conduct home health risk assessments for Medicare Advantage plan members in Leon County, FL. This role provides the flexibility to create your own schedule with shifts available any day of the week. Assessments will be performed in patients' homes throughout the region, with each evaluation taking approximately 45 minutes. Whether for annual health assessments or post-discharge visits, this position focuses exclusively on risk evaluation and education no treatment planning or prescription services required.

Schedule:

• Flexible shifts available Monday through Sunday
• Create your own schedule
• Weekly or bi-weekly minimum hour commitments

Key Responsibilities:

• Complete annual home health risk assessments for Medicare Advantage plan members
• Evaluate prior and current medical history, chronic conditions, and medications
• Discuss medical conditions and medications with patients to ensure understanding
• Identify gaps in care and provide clear recommendations for follow-up
• Travel to member residences across Tallahassee, Woodville, Midway, and other Leon County areas
• Maintain timely, accurate documentation of each encounter

Qualifications:

• Active Nurse Practitioner license in Florida
• Board certification via AANP or ANCC
• MSN degree from an accredited nursing program
• Current BLS (CPR) certification
• At least 6 months of recent NP experience preferred
