This per diem position allows you to make a tangible difference for Medicare Advantage membersdelivering quality evaluations directly in patients homes across Quincy, Chattahoochee, Havana, and surrounding communities.
Schedule:
- Flexible shifts available Monday through Sunday
- Create your own scheduling preferences
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Complete annual home health risk assessments for Medicare Advantage plan members
- Evaluate prior and current medical history, existing conditions, and medication regimens
- Offer education to patients about their health conditions and medication
- Identify gaps in care, providing actionable recommendations for improvement
- No need to create treatment plans or prescribe medications
Qualifications:
- Current Nurse Practitioner license (FL)
- Board certification through AANP or ANCC
- Active BLS (CPR) certification
- MSN from an accredited nursing school
- At least 6 months of recent Nurse Practitioner experience preferred
In this vital community-based role, each home visit typically takes around 45 minutes, with nature of visits ranging from annual health reviews to post-discharge assessments. Youll have autonomy and flexibility, with the added benefit of a per-evaluation compensation structure$80 to $90 per assessment plus a mileage stipend and administrative time.
