Schedule:

  • Flexible shifts available Monday through Sunday
  • Design your own schedule to align with personal and professional commitments
  • Minimum hourly commitment weekly or bi-weekly

Key Responsibilities:

  • Perform comprehensive home health risk assessments for Medicare Advantage plan members across Leon County
  • Assess each patients prior/current medical history, chronic conditions, and medication regimen
  • Deliver tailored education on medical conditions and medication management to support patient understanding and compliance
  • Identify gaps in care and offer recommendations to enhance patient outcomes
  • No requirements to develop treatment plans or prescribe medications

Qualifications:

  • Active Nurse Practitioner License in Florida
  • Current Basic Life Support (BLS/CPR) certification
  • Board Certification with AANP or ANCC
  • Master of Science in Nursing (MSN) from an accredited institution
  • At least 6 months of recent Nurse Practitioner experience preferred
