Step into a vital role providing comprehensive home health risk assessments to Medicare Advantage plan members throughout Leon County, FL. As a Nurse Practitioner specializing in home health, you will bring high-quality care directly to patients in a flexible, per diem settingserving areas such as Tallahassee, Woodville, and Midway.
Schedule & Hours:
- Flexible shifts available Monday through Sunday
- Create your own schedule to suit your lifestyle
- Minimum weekly or bi-weekly hourly commitment required
Core Responsibilities:
- Conduct thorough annual and post-discharge health assessments in patients homes
- Evaluate prior and current medical history, ongoing medical conditions, and medication use
- Offer clear education and guidance about medical conditions and prescribed medications
- Recognize and address care gaps, providing actionable recommendations
- Documentation and assessment focusno developing treatment plans or prescribing medications required
Qualifications & Experience:
- Current, unrestricted Nurse Practitioner license in Florida
- Active BLS CPR certification
- Board Certification through AANP or ANCC
- Minimum of 6 months recent experience as a Nurse Practitioner preferred
- Master of Science in Nursing (MSN) from an accredited program
