This role is ideal for an experienced Nurse Practitioner, passionate about holistic, patient-centered care, and desiring flexibility and autonomy in their work schedule.
Schedule:
- Flexible shifts available Monday through Sunday
- Design your own schedule to align with personal and professional commitments
- Minimum hourly commitment weekly or bi-weekly
Key Responsibilities:
- Perform comprehensive home health risk assessments for Medicare Advantage plan members across Leon County
- Assess each patients prior/current medical history, chronic conditions, and medication regimen
- Deliver tailored education on medical conditions and medication management to support patient understanding and compliance
- Identify gaps in care and offer recommendations to enhance patient outcomes
- No requirements to develop treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner License in Florida
- Current Basic Life Support (BLS/CPR) certification
- Board Certification with AANP or ANCC
- Master of Science in Nursing (MSN) from an accredited institution
- At least 6 months of recent Nurse Practitioner experience preferred
