Step into a meaningful per diem opportunity as an experienced Nurse Practitioner specializing in home health. In this flexible role, you will conduct in-home risk assessments for Medicare Advantage plan members. Assignments are in various neighborhoods of Putnam County, including areas such as Palatka, Interlachen, and Crescent City. Your visits provide valuable insight into patient well-being and foster education and support for long-term health.
Schedule:
- Flexible shifts available Monday through Sunday

- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Perform annual home health risk assessments for Medicare Advantage plan members
- Collect and evaluate information on medical history, health conditions, and medication use
- Educate members about their diagnoses and prescribed treatments
- Recognize gaps in care and offer practical recommendations for resolution
- Note: This position does not require the creation of treatment plans or prescription of medications
Qualifications:
- Active and unrestricted Nurse Practitioner license in Florida
- Board Certification from AANP or ANCC
- Current Basic Life Support (CPR/BLS) certification
- MSN degree from an accredited nursing program
- At least 6 months of recent nurse practitioner experience is preferred
