Job Description

Take on a key role delivering essential care by conducting home health risk assessments for Medicare Advantage plan members. This per diem position focuses on visits in the Putnam County, FL areaincluding Palatka, Interlachen, and Crescent Cityensuring each patient receives a thorough, personalized evaluation. Each visit typically lasts about 45 minutes and may involve an annual assessment or a post-discharge review. Candidates will have the flexibility to create a personalized work schedule and may be scheduled in any of the designated locations during individual shifts.

Schedule:

  • Flexible shifts available seven days a week
  • Set your own hoursdesign your schedule to fit your lifestyle
  • Commit to a minimum weekly or bi-weekly number of hours, as agreed upon

Key Responsibilities:

  • Complete annual health risk assessments in-home for Medicare Advantage members
  • Review both previous and current medical history, existing conditions, and medication intake
  • Educate patients about their health conditions and medications
  • Detect gaps in care, offering recommendations as needed
  • No development of treatment plans or medication prescribing required

Qualifications:

  • Active Nurse Practitioner license in Florida
  • Current CPR certification (BLS)
  • Board Certification through AANP or ANCC
  • Six months of current NP experience preferred
  • MSN from an accredited nursing program
