Job Description

Deliver vital healthcare services directly to patients homes as a Nurse Practitioner specializing in home health risk assessments in Flagler County, FL. This per diem opportunity allows you to manage your own flexible schedule while leveraging your expertise to make a significant impact on patients enrolled in Medicare Advantage plans.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Autonomy to design your own schedule
  • Minimum commitment required, either weekly or bi-weekly

Key Responsibilities:

  • Perform comprehensive annual home health risk assessments for Medicare Advantage members
  • Assess prior and current medical histories, chronic conditions, and medication usage through thorough patient evaluations
  • Offer education to patients about their medical conditions and treatment regimens
  • Identify care gaps and recommend interventions to improve health outcomes
  • No need to develop treatment plans or prescribe medications

Qualifications:

  • Current Nurse Practitioner license in Florida
  • Active BLS/CPR certification
  • Board Certification via AANP or ANCC
  • MSN degree earned from an accredited nursing program
  • Minimum of 6 months recent Nurse Practitioner experience preferred

Your commitment will include traveling within the Flagler County area, including locations such as Palm Coast, Flagler Beach, and Bunnell. Each evaluation is expected to last about 45 minutes, focusing on providing high-quality care and insightful recommendations for each patient. Compensation for this role includes $80-$90 per completed evaluation, as well as a mileage stipend and compensation for administrative time.
