Embark on a rewarding role as a Nurse Practitioner delivering home health risk assessments for Medicare Advantage members across Darlington County, including Hartsville, Darlington, and Lamar. Schedule:
- Choose your own shifts, Monday through Sunday
- Set a schedule that fits your lifestyle
- Fulfill a minimum hourly commitment weekly or bi-weekly
Key Responsibilities:
- Perform comprehensive home health risk assessments (45-minute visits) for Medicare Advantage plan members
- Review and evaluate prior and current medical histories, chronic conditions, and current medications
- Educate patients on health conditions and prescribed medications, supporting their understanding and wellness
- Identify any gaps in care, providing actionable recommendations
- Conduct annual health assessments and post-discharge visits as needed
- No requirement for prescribing medications or developing new treatment plans
Qualifications:
- Active and unrestricted Nurse Practitioner license in South Carolina
- Board Certification through AANP or ANCC
- Current BLS/CPR certification
- MSN degree from an accredited nursing program
- Minimum 6 months of current Nurse Practitioner experience preferred, but not required
This role offers $80$90 per evaluation, along with a mileage stipend and compensation for administrative time, ensuring your expertise and travel are valued and supported.
