Deliver high-quality care in a rewarding per diem role as a Nurse Practitioner providing home health risk assessments for Medicare Advantage plan members across Pittsburg County, including Hartshorne, Krebs, and Quinton. This position offers remarkable flexibilitychoose your shifts between Monday and Sunday and decide your own schedule to fit your lifestyle.
Schedule:
- Flexible shifts available any day of the week (MondaySunday)
- Set your own work schedule
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Conduct comprehensive in-home health risk assessments for Medicare Advantage members
- Review and evaluate prior and current medical history, assess medical conditions, and verify medication histories
- Educate patients and their families about ongoing medical conditions and medication regimens
- Identify care gaps and provide actionable recommendations for improvement
- No requirement to prescribe medication or develop treatment plans
Qualifications:
- Active Nurse Practitioner license in Oklahoma
- Board Certification through AANP or ANCC
- Current BLS/CPR certification
- MSN from an accredited nursing school
- At least 6 months of recent Nurse Practitioner experience preferred
