Make a meaningful impact on patient care as a nurse practitioner performing home health risk assessments for Medicare Advantage plan members. This is a per diem opportunity, offering a high degree of flexibility with scheduling in Hartford County and surrounding areas.
Schedule:
- Flexible shifts available Monday through Sunday
- Set your own hours to create a personalized schedule
- Minimal required commitment on a weekly or bi-weekly basis
Essential Qualifications:
- Active Nurse Practitioner License in Connecticut
- Board Certification via AANP or ANCC
- Current CPR certification (BLS)
- Master of Science in Nursing (MSN) from an accredited program
- Minimum of 6 months of recent nurse practitioner experience preferred
Key Responsibilities:
- Conduct 45-minute home health risk assessments for Medicare Advantage members
- Assess medical, medication, and health history during annual evaluations or post-discharge visits
- Deliver patient education about medical conditions and prescribed medications
- Identify care gaps and make appropriate, actionable recommendations
- No responsibility for treatment plan creation or prescription writing
