Make a meaningful impact on patient care as a nurse practitioner performing home health risk assessments for Medicare Advantage plan members. This is a per diem opportunity, offering a high degree of flexibility with scheduling in Hartford County and surrounding areas.

Schedule:

• Flexible shifts available Monday through Sunday
• Set your own hours to create a personalized schedule
• Minimal required commitment on a weekly or bi-weekly basis

Essential Qualifications:

• Active Nurse Practitioner License in Connecticut
• Board Certification via AANP or ANCC
• Current CPR certification (BLS)
• Master of Science in Nursing (MSN) from an accredited program
• Minimum of 6 months of recent nurse practitioner experience preferred

Key Responsibilities:

• Conduct 45-minute home health risk assessments for Medicare Advantage members
• Assess medical, medication, and health history during annual evaluations or post-discharge visits
• Deliver patient education about medical conditions and prescribed medications
• Identify care gaps and make appropriate, actionable recommendations
• No responsibility for treatment plan creation or prescription writing
