Job Description

Step into an impactful role providing comprehensive care through home health risk assessments for Medicare Advantage plan members. This per diem opportunity allows professionals to deliver high-quality patient evaluations in various locations throughout Delaware County, with assignments in areas such as Muncie, Yorktown, and Chesterfield. Each assessment, typically lasting about 45 minutes, may cover annual health evaluations or post-discharge visits tailored to the needs of individuals in their home environment.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Create your own schedule
  • Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Perform home health risk assessments for Medicare Advantage members
  • Review prior and current medical histories, medical conditions, and medication use
  • Provide patient education regarding health conditions and medications
  • Detect care gaps and offer professional recommendations to address them
  • No treatment planning or prescribing responsibilities required

Qualifications:

  • Current Nurse Practitioner license in Indiana
  • Up-to-date CPR (BLS) certification
  • Board Certification via AANP or ANCC
  • Masters degree in Nursing (MSN) from an accredited institution
  • Minimum of 6 months NP experience preferred
