Seeking a driven and compassionate Nurse Practitioner to deliver home health risk assessments on a per diem basis. In this hands-on clinical opportunity, you will serve Medicare Advantage plan members throughout Grant County, IN, including Marion, Gas City, and Upland. With the autonomy to structure your schedule, you'll enjoy flexible shifts and the ability to set your own hours.
Schedule:
- Flexible shifts available between Monday and Sunday
- Design your own work calendar
- Opportunities for minimum weekly or bi-weekly hour commitments
Key Responsibilities:
- Conduct comprehensive, standardized home health risk assessments (approximately 45 minutes each) for Medicare Advantage plan members
- Review patients prior and current medical histories, existing medical conditions, and medication profiles
- Educate patients on medical conditions and proper medication usage
- Identify and address gaps in care, making suggestions to resolve any issues
- Perform visits as annual health evaluations or post-discharge assessments
- No requirement to create treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in Indiana
- Current BLS (CPR) certification
- Board certification from AANP or ANCC
- MSN degree from an accredited nursing school
- Minimum six months of experience working as a Nurse Practitioner preferred
You will be compensated $80-$90 per evaluation, with an additional mileage stipend and administrative time pay for your professional services.
