As an experienced Nurse Practitioner, you will perform home health risk assessments for Medicare Advantage plan members in Grant County, IN, including Marion, Gas City, and Upland. Enjoy the flexibility of a schedule you design, working with a range of patient cases including annual health assessments and post-discharge visits. Each appointment averages 45 minutes, allowing you to provide focused, high-quality care in the comfort of the patients home.
Schedule:
- Flexible shifts available between Monday and Sunday
- Make your own schedule to fit your lifestyle
- Choose a minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Complete annual home health risk assessments for a Medicare Advantage health plan
- Review and assess prior/current health history, medical conditions, and medication usage
- Offer education on disease management and best practices for medications
- Identify care gaps and provide recommendations for follow-up or resolution
- No requirement to develop new treatment plans or prescribe medication; focus is on assessment and education
Qualifications:
- Active Nurse Practitioner licensure in Indiana
- Board Certified via AANP or ANCC
- Current CPR certification (BLS)
- MSN degree from an accredited school of nursing
- At least 6 months of current NP experience preferred
