Job Description

Take on a rewarding per diem role delivering high-quality care in a home health setting. As a Nurse Practitioner, you will conduct home health risk assessments for Medicare Advantage plan members, visiting clients in their homes throughout Delaware County, including areas such as Muncie, Yorktown, and Chesterfield. Each assessment averages about 45 minutes, providing a supportive experience for memberswhether for annual reviews or post-discharge follow-ups. Enjoy flexibility to create your own schedule with shifts offered any day of the week.

Schedule:

  • Choose your own hoursshifts available Monday through Sunday
  • Flexible scheduling to meet weekly or bi-weekly minimums

Key Responsibilities:

  • Complete thorough home health risk assessments for Medicare Advantage plan members
  • Review patients prior and current medical history, conditions, and medications
  • Deliver patient-centered education about medical conditions and medications
  • Identify gaps in care and propose recommendations for resolution
  • No treatment plans or prescribing requiredfocus on assessment and education

Qualifications:

  • Active Nurse Practitioner license in Indiana
  • Current CPR certification (BLS)
  • MSN degree from an accredited school of nursing
  • Board certification through AANP or ANCC
  • At least 6 months of NP experience preferred
