Deliver comprehensive patient care as a Nurse Practitioner specializing in home health assessments. This per diem opportunity offers autonomy, allowing you to schedule flexible shifts between Monday and Sunday, and tailor your workload based on minimum weekly or bi-weekly hourly commitments. Youll travel to homes within Boone County, completing risk assessments for Medicare Advantage plan members residing in areas such as Harrison, Diamond City, and Bergman. Each visit typically lasts about 45 minutes and may include annual health assessments or post-discharge evaluations.
Key Responsibilities:
- Conduct annual risk assessments for a Medicare Advantage health plan in patients homes
- Evaluate prior/current medical history, health conditions, and medication profiles
- Provide thorough education on medical issues and medication regimens
- Identify care gaps and make actionable recommendations for follow-up care
- Visits focus on evaluationno treatment planning or prescription management required
Schedule:
- Flexible shifts available any day of the week (MondaySunday)

- Meet a minimum weekly or bi-weekly commitment as you choose
Qualifications:
- Active Nurse Practitioner license in AR
- Board Certification through AANP or ANCC
- MSN from an accredited nursing program
- Current CPR (BLS) certification
- At least 6 months of recent NP experience preferred
