Make a meaningful impact in the lives of patients by performing home health risk assessments for Medicare Advantage members across Boone County. This per diem Nurse Practitioner opportunity offers high flexibility, autonomy, and the ability to build your own schedule with shifts available Monday through Sunday. Youll visit members in Harrison and other communities in Boone County, such as Diamond City and Bergman, as part of a dynamic outreach team.
Schedule:
- Flexible shifts available Monday through Sunday
- Self-scheduling: build your own hours
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Complete comprehensive home health risk assessments, averaging about 45 minutes per visit
- Review and evaluate current and previous medical and medication history
- Educate members about their medical conditions and prescribed medications
- Identify gaps in care and recommend steps for improved health outcomes
- Conduct annual or post-discharge visits as required
- No prescribing or developing treatment plans required
Qualifications:
- Active Nurse Practitioner license in Arkansas
- Board Certification through AANP or ANCC
- Current CPR/BLS certification
- MSN degree from an accredited program
- At least 6 months of recent Nurse Practitioner experience is preferred
