Schedule:
- Flexible shifts available Monday through Sunday
- Design your own work schedule
- Option for minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Conduct comprehensive annual risk assessments in patients' homes across Boonville, Wooldridge, Blackwater, Otterville, and other Cooper County communities
- Review and evaluate patients current and past medical history, ongoing medical conditions, and medication regimens
- Educate patients about their health conditions and medications, ensuring they understand their care
- Identify gaps in care and provide practical recommendations to improve health outcomes
- No requirement to develop treatment plans or prescribe medications
Qualifications:
- Unencumbered Nurse Practitioner license in Missouri
- Board certification through AANP or ANCC
- Current BLS/CPR certification
- MSN degree from an accredited nursing program
- At least 6 months of Nurse Practitioner experience preferred
