Seeking a skilled Nurse Practitioner with home health experience to provide comprehensive home health risk assessments for Medicare Advantage members across Pottawatomie County. This per diem opportunity offers exceptional flexibilitytailor your schedule with shifts available any day of the week.

Schedule:

• Flexible shifts available Monday through Sunday
• Build your own schedule
• Minimum weekly or bi-weekly hour commitment required

This role focuses on conducting thorough risk assessments in patient homes, supporting high-quality healthcare delivery to the Shawnee, Tecumseh, McLoud, and surrounding communities.

Key Responsibilities:

• Complete annual and post-discharge health risk assessments for Medicare Advantage members in their homes
• Review prior and current medical histories, conditions, and medication use
• Educate members regarding their health conditions, medications, and wellness strategies
• Detect gaps in care and offer recommendations for improvement
• No prescribing medications or developing treatment plans required

Qualifications:

• Current and active Nurse Practitioner license in Oklahoma
• Valid CPR certification (BLS)
• Board Certification via AANP or ANCC
• Master of Science in Nursing (MSN) from an accredited institution
• Minimum of 6 months recent NP experience preferred
• Experience in home health settings desirable
• Excellent communication and organizational skills
